Psychiatric Care Services

At KHS, the first step in psychiatric care begins with a comprehensive psychiatric evaluation. Performed by a psychiatrist, this initial examination is both psychological and medical in nature. Physicians at KHS may ask for your physical, behavioral, relationship and cognitive histories, order lab tests or other medical exams. Once the comprehensive assessment is complete, our psychiatrists will work to formulate an assessment and recommend a treatment plan in conjunction with other members of the patient’s treatment team for continually treatment.

Case Management

Case management involves the collaborative process aimed at client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation. Its underlying premise lies in the belief that everyone benefits when clients reach their optimum level of wellness, management, and functional capability. As recognized experts in the care coordination team, case managers link clients with providers and resources within health and human services and therapeutic care settings. They ensure that the care being provided is safe, effective, timely, and centered on the client.

The Whole-Person Approach

While traditional primary care physicians play a unique role in addressing a patient’s physical needs, we recognize the importance of integrating mental health services into primary care. Mental health is often measured by the performance of mental function, fulfilling relationships with other people, and the ability to adapt, change and cope with adversity. We work to incorporate the social, emotional and behavioral aspects of an individual’s life and expand our services from treatment to health promotion and early intervention. This integration promotes optimal social and emotional development, and our providers assume greater responsibilities for the coordination of care of our patients and play a critical role in the screening and ongoing management of a patient’s emotional and behavioral problems.

Patient Follow Up

Additionally, the team follows up with the patients within 48 hours, 15 days, and 30 days. These phone calls or visits, enable them to resolve any issues or questions patients may have regarding their medications. The goal of the program is to prevent any readmission within 30 days.

Psychological Rehab

Medicaid defines Psychosocial Rehab (PSR) are interventions designed to reduce psychosocial dysfunction (i.e., interpersonal cognitive, behavioral development, etc.) and restore recipients to their highest level of functioning. PSR services target psychological functioning within a variety of social settings. Both BST or PSR services are not offered as standalone interventions, but are incorporated into the patient’s plan of care to reduce or overcome obstacles to success in attaining their short and long-term goals.

Transportation

KHS provides transportation services to our patients as a way of eliminating or minimizing transportation issues as reasons for missing critical physician, psychiatry and therapy appointments that are critical to obtain and maintain medical, psychological and behavioral balance.
